About Frameworks | Why is disease management important?
Disease management is a system of coordinated heathcare interventions and communications for people with chronic medical conditions.2 Disease management focuses on prevention of complications through the use of evidence-based care.2 The main goals of these programs are to reduce symptoms of a condition or to keep them from getting worse.3 Disease management programs (DMPs) also aim to reduce healthcare costs and improve quality of life. This is significant because 90% of the nation’s $3.3 trillion in annual healthcare costs is attributed to people with chronic and mental health conditions.4 DMPs empower patients, working with their healthcare providers, to manage their disease.2
